Incident Report requests are processed within seven (7) business days upon receipt. The cost is $5.00 per report. Accepted forms of payment are: Check, Money Order, VISA, MasterCard, or Discover. Cash is not accepted.

If you would like to pick up an incident report in person at our headquarters (address above), please send your completed form, with the "I would like to pick up the report" option selected, via email to This email address is being protected from spambots. You need JavaScript enabled to view it. or by fax to (916) 859-3701. You may also contact our records clerk at (916) 859-4161 to place your order. Please make your request well in advance of your intended pick up time to ensure the report is available. Once the report is ready for pick up, you will receive a confirmation phone call and/or email.
Our office hours are Monday-Friday, 8 AM-12 PM and 1 PM-5 PM.
